Highland Health & Rehabilitation is a 44-bed skilled nursing and rehabilitation facility dedicated to delivering high-quality, resident-centered care. We’re looking for a compassionate and dependable Licensed Practical Nurse (LPN) to join our clinical team and help support residents through recovery, wellness maintenance, and long-term care needs.

What You'll Do
Under the direction of the RN, the Licensed Practical Nurse (LPN) provides individualized nursing care for residents in a skilled nursing setting. Key responsibilities include:
Deliver direct nursing care within scope of practice
Collaborate with RNs using the SBAR communication framework
Participate in resident assessments and care planning
Monitor resident conditions and communicate changes to the care team and families
Administer medications and treatments as ordered
Review and implement physician orders per regulatory requirements
Support medication safety through accurate documentation and monitoring
Supervise Medication Aides/Technicians as applicable
Assist with admissions, discharges, and consultant visits
Participate in emergency response, including CPR if needed
Maintain accurate and timely clinical documentation
Support pharmacy recommendations and medication reviews
Participate in quality assurance and medical record audits
Maintain regulatory compliance with company, state, and federal standards
What You Bring
Active WA state LPN license
Graduate of an accredited nursing program
Current CPR certification
Minimum 1 year supervisory experience in healthcare preferred
At least 6 months long-term care experience preferred
Strong understanding of HIPAA compliance
Ability to respond effectively in emergency situations
Excellent communication skills with residents, families, and interdisciplinary teams
Commitment to resident dignity, safety, and quality outcomes
