About the Role
Nutritious Lifestyles is seeking a compassionate and patient-focused Community Nutritionist to provide individualized nutrition care and education to patients in a community healthcare setting.
This full-time, community-based position focuses on helping people improve their health and quality of life through the power of Food as Medicine. Working directly with individuals and families, you will empower community members to make sustainable lifestyle changes, prevent disease, and better manage chronic and infectious health conditions through personalized nutrition care.
What You'll Do
* Provide one-on-one nutrition counseling and medical nutrition therapy to patients.
* Conduct nutrition assessments and develop individualized nutrition care plans.
* Educate patients on healthy eating habits, disease prevention, and lifestyle modifications.
* Support patients managing chronic conditions such as diabetes, hypertension, cardiovascular disease, obesity, and kidney disease.
* Provide nutrition guidance for patients affected by infectious diseases and other health conditions.
* Monitor patient progress and adjust nutrition interventions as needed.
* Collaborate with physicians, nurses, case managers, and other healthcare professionals to coordinate patient care.
* Document nutrition assessments, recommendations, and follow-up care in accordance with organizational standards.
* Promote health, wellness, and disease prevention through community-based nutrition services.
Who Should Apply:
* Registered Dietitian Nutritionist (RDN) or LDN License.
* Experience in community nutrition, public health, outpatient counseling, or clinical nutrition preferred.
* Strong communication, counseling, and eagerness to learn.
* Passion for improving health outcomes through Food as Medicine.
